Join a team that prioritizes growth, open communication, and the tools you need to make a meaningful impact in the lives of others.Location: San Diego Central and East About The OpportunityThe Speech-Language Pathologist (SLP) evaluates, plans, and provides speech therapy services to adult patients in their homes in accordance with physician orders and the Plan of Care (POC).ResponsibilitiesPerform comprehensive speech, language, cognitive, and swallowing evaluations.Assess the home environment and recommend adaptive equipment as needed.Develop and update individualized Plans of Care in collaboration with the physician and care team.Provide skilled speech therapy interventions using evidence-based techniques.Monitor patient progress and modify treatment plans accordingly.Document visits accurately and timely in the electronic medical record.Communicate patient status to physicians, case managers, and interdisciplinary team members.Educate patients, families, and caregivers on therapy goals and home programs.Participate in case conferences, in-services, and team meetings.Follow infection control and HIPAA privacy standards.Required QualificationsActive Speech-Language Pathologist license in the state of practice.Master’s or Doctorate degree in Speech-Language Pathology.Current BLS/CPR certificationValid driver’s license, insured vehicle, and ability to travel to patient homesAbility to work independently and manage schedule effectivelyExperience working with adult/geriatric populationsMinimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting.Why Join us?We are committed to our employees' well-being, offering a comprehensive employee benefits package that includes, but is not limited to:Competitive salary and comprehensive benefits package (medical, dental, vision, life, AD&D, disability, EAP, 401(K) with 4% matchPaid time off and paid holidaysMileage reimbursement or company vehicle (role dependent), plus fuel cardEmployee referral program and opportunities for growth and advancementThis position is based on a Pay per visit compensation model.Pay per visit ranges from $95 to $120 visit commensurate with experience and depending on type of visit.
