Community Care Coordinator

The Patient Care Navigator (PCN) serves as a primary liaison between healthcare providers and individuals recovering from limb loss. The PCN delivers superior patient care and customer service via proactive intervention, communication, and providing access to emotional support and educational resources.  The PCN should be equipped with a thorough understanding of prosthetics, orthotics, and the medical/healthcare field.  The PCN offers patient consultation and guidance from the initial patient referral to prosthetic delivery, serving as a single point of contact as they transition through the continuum of care.  Working closely with Hanger prosthetic personnel, office staff, and Business Development Mangers this position holds responsibility for the day-to-day management of, and...
