PRN Occupational Therapist (OTR)

Peninsula Post Acute is seeking a dedicated and compassionate PRN Occupational Therapist (OTR) to join our rehabilitation team. This position offers the opportunity to make a meaningful impact on residents' lives by helping them achieve greater independence, improve functional abilities, and enhance overall quality of life in a skilled nursing and post-acute care setting.

Compensation: $64–$66+ per hour, depending on experience (DOE)

Position Summary

The Staff Occupational Therapist evaluates and treats residents, communicates with families, physicians, and interdisciplinary team members, and maintains accurate documentation of services provided. The Occupational Therapist also conducts staff training and in-service education programs to support quality resident care and rehabilitation outcomes.

Essential Duties and Responsibilities

  • Evaluate residents to determine occupational therapy needs, functional limitations, and rehabilitation potential in accordance with physician orders and facility policies.

  • Develop, implement, and modify individualized treatment plans to improve residents' functional independence, safety, and participation in activities of daily living (ADLs).

  • Provide skilled occupational therapy interventions and monitor resident progress toward established goals.

  • Assist the nursing department with training and support of Restorative Aides.

  • Supervise Certified Occupational Therapy Assistants (COTAs) in direct resident care and resident-related activities in accordance with state practice regulations.

  • Communicate regularly with physicians, nursing staff, interdisciplinary team members, residents, and families regarding treatment progress, goals, recommendations, and discharge planning.

  • Participate in Resident Care Conferences, Utilization Review meetings, Rehabilitation meetings, and other interdisciplinary meetings as needed.

  • Provide in-service education and training programs for facility staff as appropriate.

  • Complete timely, accurate, and compliant documentation in accordance with facility policies, regulatory requirements, and third-party payer guidelines.

  • Maintain a safe treatment environment by adhering to infection control, safety, and equipment maintenance standards.

  • Educate residents, families, and caregivers regarding treatment goals, home exercise programs, maintenance programs, safety techniques, and fall prevention strategies.

  • Recommend and facilitate the ordering of durable medical equipment and adaptive devices to promote resident independence, including splints, adaptive equipment, assistive devices, elevated toilet seats, and grab bars.

  • Maintain confidentiality of resident information and company records in accordance with applicable laws and facility policies.

  • Report equipment concerns promptly to ensure rehabilitation equipment remains safe and operational.

  • Contribute to quality assurance initiatives, performance improvement activities, and rehabilitation program development.

  • Establish and maintain positive, professional relationships with residents, families, staff, and healthcare professionals.

Supervisory Responsibilities

  • Supervise Certified Occupational Therapy Assistants (COTAs), rehabilitation aides, and therapy students.

  • Provide guidance, clinical oversight, and mentorship to support quality resident care and professional development.

  • Ensure delegated services are provided in accordance with state practice acts and facility standards.

Qualifications

  • Current Occupational Therapist (OTR) license in the State of California.

  • Graduate of an accredited Occupational Therapy program.

  • Current CPR certification preferred.

  • Experience in skilled nursing, post-acute care, or rehabilitation settings preferred.

  • Strong clinical assessment, documentation, communication, and organizational skills.

  • Ability to work independently and collaboratively within an interdisciplinary team.
