You drive housing stability, helping clients#secure and maintain safe housing by navigating applications, connecting them to benefits, and collaborating with housing teams to remove barriers. You commit to trauma-informed care, building#trust through empathy, and a#strengths-based, non-judgmental approach, guiding clients to set meaningful goals and facilitating peer and family support groups. You connect clients to critical resources, linking#individuals to medical, educational, vocational, and community services, ensuring coordinated, wraparound support tailored to each client#s needs. You manage care with precision and responsiveness, maintaining#consistent client contact,#applying best practice in crisis situations, and handling#documentation and compliance while providing dependable support when it matters most. HOW WILL YOU MAKE AN IMPACT? As the#Case Manager and Peer Advocate, you will provide comprehensive and intensive case management services to children and adults with severe and persistent mental illness who may have a history of homelessness and/or history of hospitalizations. Case management services include linkage to community resources such as housing, medical, educational, and vocational services. In addition, you will provide#rehabilitation services that focus on improving daily functioning through an integrated recovery approach that improves daily living skills and supports consumers in meeting their goals. PRIMARY DUTIES#AND ESSENTIAL FUNCTIONS# Provide targeted case management and mental health rehabilitation services to clients, including linking them to#appropriate community#resources, as well as#assisting#with money management, skills training, and independent living skills to improve their functioning. Assist#clients who may be experiencing homelessness in obtaining resources for housing within the community.#Assist#in obtaining benefits to pay for housing where possible.#Assist#clients in completing necessary housing applications where#appropriate#and work collaboratively with Housing Specialist and administrative team on updates to housing placements to ensure#timely#notification of changes/updates to housing status. Work as a collaborative team member with inter and intra-agency staff to#assist#in stabilizing clients. Maintain at least weekly#face-to-face#contact with assigned clients. Ensure clients are involved in#establishing#goals in the areas of living arrangements, social relationships, constructive use of leisure time, productive activity, medical care, and other areas#impacting#their functional impairments. Responsible for documentation including Log Sheets, DMH paperwork, Progress Notes, and Designated Outcome Measures as well as creating Behavior Charts and schedules for client/family as needed. Meet required service delivery expectations. Meet required agency note submission deadline. Comply with#HIPPA#requirements#for privacy and access of records. Provide written translation services. Demonstrate cultural sensitivity to each client/family and individual needs and customs as appropriate. Actively#participate#and attend in-service presentations, mandatory meetings, and trainings as scheduled. Provide self-help groups and peer-to-peer/family-to-family/parent-to-parent groups. Provide advocacy within the community on behalf of clients/families from an underserved population. Support client transportation to linkage services and other pre-approved outings, as needed. Provide on-call crisis support 24 hours / 5 days a week by phone and/or in-person if#indicated. Participate#in#the on-call weekend rotation to provide crisis support, which may include holidays. Other duties as assigned by supervisor to meet programmatic, departmental and agency needs.# MINIMUM QUALIFICATIONS AND COMPETENCIES Education /Background: BS/BA degree highly preferred. Equivalent combinations of education and experience may be considered to meet qualifications for this position. 2 years# experience in case management preferred. Lived mental health experience preferred. CA Driver License required and clean driving record for insurance to assist with client transport or other linkage outings as needed. Bilingual Spanish speaking required. Other Requirements: Knowledge of community health care and community services. Knowledge of case management methods, procedures, and documentation. Experience working with multiethnic/racial clients and communities. Ability to adhere to agency and clinic policy and procedures. Ability to maintain confidentiality of records and information. Ability to work well as part of the case manager team. Ability to communicate effectively, both orally and in writing. Ability to prepare effective written materials including correspondence and reports. Ability to be flexible and multitask, handling several tasks simultaneously.
