Outpatient Pulmonary Advanced Practice Provider (APRN/PA)

Full-time Naples, Florida

The Outpatient Pulmonary Advanced Practice Provider (APRN or Physician Assistant) delivers comprehensive, patient-centered care within a pulmonary specialty clinic, in collaboration with the supervising pulmonologist and interdisciplinary care team. The APP must maintain an active, unrestricted Florida license in good standing.

This role focuses exclusively on the outpatient management of patients with acute and chronic respiratory conditions. The APP is responsible for evaluating, diagnosing, and treating pulmonary diseases such as COPD, asthma, pulmonary fibrosis, interstitial lung disease, pulmonary nodules, and sleep-related breathing disorders.

The APP will perform detailed clinical assessments, develop and manage treatment plans, and monitor disease progression to optimize outcomes and prevent exacerbations, emergency department visits, and hospitalizations. The role includes interpretation of pulmonary diagnostics such as pulmonary function tests (PFTs), imaging studies, and laboratory results.

The APP will provide education to patients and caregivers on disease management, inhaler technique, oxygen therapy, smoking cessation, and lifestyle modifications. Care coordination with referring providers, primary care, and ancillary outpatient services is a key responsibility to ensure continuity of care.

This position emphasizes proactive, preventive care management in an outpatient setting, including addressing quality measures, HCC coding, and value-based care initiatives. The APP is responsible for accurate and timely documentation, billing, and adherence to CMS and organizational guidelines.

How You Will Make an Impact & Requirements

Essential Duties and Responsibilities

  • Provide outpatient pulmonary care in collaboration with the supervising pulmonologist.
  • Perform comprehensive respiratory assessments, including history, physical exam, and diagnostics review.
  • Diagnose and manage chronic and acute pulmonary conditions using evidence-based guidelines.
  • Interpret and act on pulmonary diagnostics (PFTs, chest imaging, labs, sleep studies as applicable).
  • Develop, implement, and adjust individualized treatment plans.
  • Manage chronic lung disease to prevent exacerbations and reduce hospital utilization.
  • Educate patients on inhaler use, medication adherence, oxygen therapy, smoking cessation, and disease self-management.
  • Identify and refer patients to appropriate outpatient services, including pulmonary rehabilitation, sleep studies, and home health.
  • Coordinate care with primary care providers and specialists to ensure seamless outpatient management.
  • Address and document quality metrics and HCCs in support of value-based care.
  • Ensure accurate, timely, and compliant documentation in the electronic medical record.
  • Support clinic workflow efficiency and collaborate with medical assistants and office staff.
  • Consult supervising physician on complex clinical cases as needed.
  • Perform other duties as assigned.

Required Knowledge and Experience

  • Active Florida APRN or PA license in good standing
  • Board Certification (APRN or PA)
  • Preferred experience in pulmonology, outpatient internal medicine, or respiratory care
  • Strong knowledge of pulmonary disease management in an outpatient setting
  • Excellent patient communication, education, and care coordination skills
