JOB SUMMARY In collaboration with patients, families (as defined by the patient) and staff across Program Specific disciplines and departments, the Registered Nurse, Care Manager - Permanent Supportive Housing is responsible for managing all aspects of patient-centered care for patients with chronic illness.
