LINKAGE ASSISTANT (Bilingual- English & Spanish or Creole)

The Linkage Assistant is responsible for providing outreach services to newly diagnosed individuals with HIV/AIDS, previously diagnosed individuals who entered treatment but are currently not receiving treatment, and previously diagnosed individuals at risk of losing care. They are responsible for conducting activities to engage and enroll HIV positive individuals into the care and treatment delivery system. Help with assisting HIV positive individuals who lack care with the re-entry process into the care and treatment delivery system, and assist HIV positive individuals who are at risk of losing care with retention and access to continuous medical care and treatment.
