Community Habilitation Counselor: Part Time Queens & Long Island

A Community Habilitation Counselor is responsible for providing services to persons with Intellectual Developmental Disabilities and Autism in accordance with the established Community Habilitation Plan (on a case by case basis). They will participate in the training and supervision of the program participant while incorporating the use of community resources. They will be responsible for accurate completion of the documentation record, monthly summary notes, and expense forms via paper and electronic formats.
