Job Summary:

We’re looking for a proactive, client-focused professional with strong communication, advocacy, and community resource navigation skills.
Provide on-site mental health support and case coordination in area schools, collaborate closely with school staff and families, develop individualized care plans, monitor client progress, and link youth to social, vocational, educational, and recreational resources to support their wellbeing.

Key Responsibilities:
· Provide on-site intervention and direct support to youth enrolled in the school-based program, delivering evidence-informed strategies that promote safety, stabilization, and skill-building.
· Develop and implement individualized treatment plans that reflect each student’s strengths, needs, and goals; update plans based on progress and team input.
· Collaborate with school personnel, families, and community partners to coordinate care, share progress, and align supports across settings.
· Monitor and document client progress, outcomes, and service needs to inform treatment adjustments and service planning.
· Advocate for clients to ensure access to mental health services, community resources, educational supports, and other needed services.
· Connect students with community agencies and resources to support social, vocational, educational, and recreational engagement.
Education, Experience, and/or Credential Qualifications:
· Several qualifying pathways: education, certification, or related experience (see options below).
· Bachelor's degree in Psychology, Social Work, Sociology, Education, Criminal Justice, Family Studies, Counseling, Recreational Therapy, Human Services, Human Development, Child Development, Gerontology, Behavioral Science, or Rehabilitation Counseling
· OR work experience in a related human services field may substitute for educational requirements; related fields include case management, residential support, developmental disability support, vocational rehabilitation, and more. Four years of qualifying experience minimum, or a combination of education and experience equaling four years, is required.
Additional Qualifications:
· Valid Driver’s License, current auto insurance, and a favorable recent driving record.
· A proactive, client-focused approach and a commitment to reducing stigma around mental health.
· Strong interpersonal, communication, and collaboration skills; ability to engage youth and families effectively.
Physical Requirements
· Ability to travel between schools and community locations (mileage reimbursement provided).
· Ability to sit, stand, and move throughout school environments; perform tasks that require typical physical activity associated with community-based youth services.
