Job Purpose 

The Certified Occupational Therapy Assistant (COTA) provides clinician‑guided at‑home occupational therapy services under the direction and supervision of a licensed Occupational Therapist. This role implements established treatment plans, delivers evidence‑based interventions focused on restoring functional independence, and supports patients through post‑operative recovery and functional rehabilitation. The COTA plays a key role in patient engagement, documentation accuracy, and continuity of care while operating within a fast‑paced, patient‑centered, technology‑enabled clinical environment. 

 

Key Responsibilities 

Essential Duties 

Clinical Care Delivery 

  • Implement occupational therapy treatment plans established by the supervising Occupational Therapist. 

  • Deliver therapy sessions focused on activities of daily living (ADLs), mobility, strength, endurance, and functional independence. 

  • Guide patients through therapeutic exercises, functional activities, and recovery protocols. 

  • Monitor patient progress and promptly report changes, concerns, or barriers to the supervising OT. 

  • Accurately and timely document patient performance, interventions, and progress toward goals. 

  • Educate patients on recovery expectations, home exercise programs, safety, and functional strategies. 

  • Reinforce patient engagement and adherence to prescribed treatment plans. 

  • Support individual and group‑based therapy sessions as assigned. 

Program Support and Operations 

  • Assist with maintaining standardized clinical workflows and documentation processes. 

  • Support implementation of functional recovery initiatives and operational procedures. 

  • Collaborate with Occupational Therapists and interdisciplinary teams to ensure continuity of care. 

  • Participate in team meetings, case reviews, and care coordination activities. 

  • Provide feedback to improve clinical operations, patient experience, and workflow efficiency. 

Technology, Compliance, and Quality 

  • Utilize telehealth platforms and digital tools to deliver high‑quality patient care. 

  • Assist patients with navigating digital therapy platforms and engagement tools. 

  • Maintain proficiency with electronic documentation systems and collaboration tools. 

  • Deliver services in accordance with state licensure laws, payer requirements, and applicable telehealth regulations. 

  • Follow established supervision requirements and collaborate closely with the supervising OT. 

  • Participate in quality assurance activities and continuous improvement initiatives. 

  • Maintain patient confidentiality and comply with HIPAA and professional standards of care. 

 

Other Functions 

  • Support clinical and operational initiatives as needed to enhance patient outcomes and care delivery. 

Performs other related duties as assigned to support operational and business needs. 

JOB QUALIFICATIONS

Skills and Abilities

Required

  • Strong understanding of functional recovery and rehabilitation principles
  • Ability to effectively implement treatment plans developed by an Occupational Therapist
  • Strong interpersonal, patient engagement, and communication skills
  • Accurate and timely clinical documentation skills
  • Strong organizational and time‑management abilities
  • Ability to work independently while collaborating within a clinical team
  • Comfort using technology platforms, digital tools, and electronic documentation systems
  • Patient‑centered mindset focused on functional outcomes and quality care
  • Adaptability in a fast‑paced healthcare environment

Preferred

  • Experience delivering care via telehealth or technology‑enabled platforms
  • Experience in digital health or innovative care delivery models

Work Experience Requirements

Required

  • Experience working under the supervision of a licensed Occupational Therapist

Preferred

  • 1–3 years of clinical experience, particularly in:
    • Post‑operative care
    • Orthopedics
    • Functional rehabilitation
    • Acute, subacute, or outpatient rehabilitation settings

Education Requirements

Preferred

  • Associate degree from an accredited Occupational Therapy Assistant program
