Job Description

The Heart Failure Nurse Practitioner (NP) is a NP who works under the direction of the Department's Administrative and Medical Directors to facilitate safe and efficient care of the Advanced Heart Failure/Cardiology patient population. The NP is responsible for the application of the nursing process, supervision of clinical practice, education, leadership, and patient care management. The NP will function as a part of inter-professional teams by communicating, planning, and implementing care directly with other healthcare professionals, including but not limited to physicians, pharmacists, office support staff, and clinical nurse specialists and nurse practitioners. This position is a Full-time Heart Failure Nurse Practitioner position that will involve supporting the Cardiovascular Heart Failure Service Line by using a team approach to efficiently evaluate and treat our Advanced Heart Failure patient population including heart transplant, LVAD, and temporary mechanical circulatory support patients. The NP will be responsible for rounding with the Advanced Heart Failure team in the ICU and the floor for both primary and consult patients. The NP will be responsible for responding to calls for consults and providing appropriate documentation including H&P's, new patient consult notes and follow-up notes. The NP will also be expected to meet all medical center requirements, including an emphasis on goal directed heart failure therapy. Some duties include admitting patients from the office to the hospital, providing patient and family education and teaching, discharge education, post-discharge phone calls, arranging inpatient and outpatient procedures, preparing nightly sign out, coordinating outpatient follow-up and other facets necessary for coordination of care. The nurse practitioner should demonstrate a knowledge of regulatory requirements in the delivery of patient care. Demonstrates critical thinking in providing care according to the standards of the profession.

Essential Duties:

  • ASSESSMENT 1. Completes an initial assessment at the beginning of the day 2. Continually reassesses Clinic Needs 3. Identifies potential barriers to efficient operational flow 4. Assesses needs of the clinic on at regular intervals 5. Ensures complete and accurate documentation.
  • PLANNING 6. Ensures timely communication patient needs to Providers
  • INTERVENTION 7. Utilizes professional, respectful, effective communication to delegate and direct activities and assignments 8. Supports the staffing and operational needs of the unit and organization 9. Ensures prompt response to service needs as appropriate
  • 10. Responsible for the routine update of required licenses and credentials as required by organization EVALUATION 11. Evaluates and modifies current practices to assure the most efficient way to assure effective operational flow
  • LEADING AND ROLE MODELING 12. Acts as a clinical resource and support 13. Keeps informed of clinical / service / organizational updates / changes 14. Models compassion, understanding, problem solving and conflict resolution skills amongst the staff to assist patients and families having difficulty coping with their illness and/or hospitalization and maintains good public relations 15. Consistently keeps current on clinical, department and medical center changes; shares information with staff members by participating in huddles and utilizing all forms of communication as directed by the department Manager 16. Models expertise through the application of evidenced-based practice in clinical decision making 17. Demonstrates enthusiasm and commitment to the service, department and organization’s mission and goals 18. Participates in departmental or organizational activities as assigned 19. Partners with Leadership in making daily staffing assignments in accordance with budgeted hours per patient day and provides rationale for any staffing variances. 20. Seeks out opportunities for professional development (e.g. certifications, nursing committees, unit based projects, research, published peer review article) 21. Effectively collaborates with other disciplines and providers to meet the needs of patients and fellow staff members
  • DIRECT PATIENT CARE ACCOUNTABILITIES – AS APPROPRIATE & APPLICABLE 22. Follows policies and procedures; demonstrates ability to access online manuals. 23. Transfers learned scientific knowledge into practical application in applying the Nursing Process. 24. ASSESSMENT Appropriately completes patient assessments
  • 25. DIAGNOSIS Interprets assessment information and formulates Nursing Diagnoses 26. PLAN – IPOC Develops Plans of Care with individualized interventions. Documents expected/projected patient outcomes. Reviews/Revises/Updates Plans of Care daily based on evaluation and interpretation of patient outcomes 27. INTERVENTION Completes patient care procedures correctly per policy at prescribed intervals. Delegates tasks based on others’ scope of practice and competency. Acts as patient advocate. 28. DOCUMENTATION Documentation is complete and correct in Electronic Health Record 29. EDUCATION Provides patient/family education as needed 30. TRANSFER AND DISCHARGE Collaborates with Case Manager and/or social work for any social needs encountered with population. 31. EQUIPMENT Utilizes patient care equipment appropriately and safely according to manufacture guidelines (e.g.) 32. COMMUNICATION Communicates effectively with all members of the healthcare team including patient and family 33. ORGANIZATION Manages time effectively. Able to organize and prioritize responsibilities. 34. OBSERVES REGULATORY AGENCY REQUIREMENTS 35. Measure based on the appropriate level of care expected: MANAGEMENT OF THE CARE OF CARDIOVASCULAR MEDICINE PATIENTS Demonstrates the ability to manage post CARDIAC PROCEDURE PATIENTS. 36. QUALITY MEASURES Identifies and participates in data collection and process improvements related to Nurse Sensitive Quality Indicators 37. Performs other duties as assigned. (Examples: Enthusiastic participation in unit activities such as process improvement efforts, preceptor, peer education; changing assignment to meet patient needs, etc.)

Required Qualifications:

  • Req Master's degree Graduate from an accredited school of Nursing (MSN).
  • Req 2 years A minimum of two years experience as a RN or NP in Cardiology required
  • Req 2 years A minimum of 2 years experience in advanced heart failure, VADS, mechanical circulatory support, ECMO management and Heart Transplant required.
  • Req Strong critical and strategic thinking, analytical and planning skills.
  • Req Organization/time management skills.
  • Excellent communication skills.


Preferred Qualifications:

  • Pref 5 years 5 years of NP experience preferred


Required Licenses/Certifications:

  • Req Registered Nurse - RN (CA Board of Registered Nursing)
  • Req Nurse Practitioner - NP (CA Board of Registered Nursing)
  • Req Nurse Practitioner Furnisher (CA DCA) Current license to furnish medications in the State of California.
  • Req DEA Certificate Drug Enforcement Agency (DEA) Certificate issued by the U.S. Department of Justice for Schedule II-V controlled substances.
  • Req NP Board Certified Possession of National Board Certification in Nurse Practitioner area of specialty accredited by Accreditation Board of Specialty Nursing (ABSNC) or National Commission for Certifying Agencies (NCCA). ◦ If work in Hospital/Acute Care Setting must have Certification from the American Nurses Credentialing Center (ANCC) or American Association of Critical-Care Nurses (AACN) as an Acute Care Nurse Practitioner (ACNP) or Adult Gerontology Acute Care Nurse Practitioner (AGACNP) upon hire. ▪ (*Grandfathered from Acute Care Certification requirement if hired/transferred prior to 12/2015.)
  • Req Basic Life Support (BLS) Healthcare Provider from American Heart Association
  • Req Advanced Cardiovascular Life Support (ACLS) Healthcare Provider from American Heart Association - *ACLS as required by the NPs Job description for NPs in the Hospital/Acute Care Setting and Ambulatory Infusion Centers (*Required for Hospital/Acute & Infusion Center Clinic settings only)
  • Req Fire Life Safety Training (LA City) If no card upon hire, one must be obtained within 30 days of hire and maintained by renewal before expiration date. (Required within LA City only)

The annual base salary range for this position is $161,500.00 - $264,160.00.
