This is an opportunity to work closely with residents, families, physicians, and an experienced nursing team to provide outstanding wound care and treatment services in a supportive long-term care environment.

What You'll Do
âś” Provide physician-ordered treatments and medication administration
âś” Monitor, assess, and document wounds and skin conditions
âś” Communicate with residents, families, physicians, PT/OT, and interdisciplinary teams
âś” Assist with care planning and treatment protocols
âś” Ensure dignity, comfort, and compassionate care for every resident
âś” Support and guide nursing staff through training and teamwork
âś” Maintain accurate documentation and compliance standards
Qualifications
âś… Current California LVN/LPN license in good standing
âś… Current CPR certification
âś… 2+ years of nursing experience preferred, especially in long-term care
âś… PCC experience preferred
âś… Strong communication and customer service skills
âś… Ability to work collaboratively with residents, families, physicians, and staff

Physical Requirements
• Ability to stand/walk frequently
