Mental Health Clinician - Intensive Case Management Team

The Mental Health Clinician on CCC’s Intensive Case Management team delivers collaborative, client-centered care for adults navigating severe mental illness, addiction, homelessness, and complex health needs. You’ll conduct assessments, develop care plans, provide counseling and crisis response, support engagement through outreach, and coordinate closely with a multidisciplinary team. This role is fast-paced, mission-driven, and focused on stabilizing clients and helping them move toward safety, health, and recovery.
