Mental Health Clinician - ACT Team

The Assertive Community Treatment (ACT) Mental Health Counselor works as a member of a multi-disciplinary behavioral health community-based team that provides clinical care within a care team providing close collaboration & coordination with clients enrolled at Old Town Recovery Center.  The ACT Mental Health Counselor is responsible for the delivery of care to enrolled clients with severe and persistent mental illness, ensuring care that is comprehensive, proactive, trauma-informed, and client-centered.
