Community Health Coordinator

The Community Health Coordinator (CHC) is an essential member of the interdisciplinary care team, working closely with the RN Manager and Medical Assistant to coordinate and support patient care. Acting as a caring neighbor and advocate, the CHC helps patients navigate environmental, social, and economic barriers to better health by providing support, resources, and encouragement toward lifestyle changes and self‑management goals. The CHC conducts in-person visits (home, hospital, or other community settings) as well as virtual visits to assess and address patients’ social and community support needs. The CHC also performs ongoing care calls to ensure timely interventions, patient engagement, and progress toward care plan goals.
