Senior Coordinator, Care Access

The Senior Care Access Coordinator delivers advanced member and provider care coordination and support to ensure timely access to behavioral health services, with a focus on posthospitalization outreach, high risk follow up, and counseling appointment coordination. This role requires greater independence, critical thinking, and judgment as it helps members understand benefits, identify providers, overcome barriers to care, and connect with appropriate clinical resources, ensuring continuity of care. The Senior Care Access Coordinator collaborates closely with clinicians, Provider Success, Medical Directors, and internal teams while documenting accurate member information to support continuity of care.
