Community Health Worker

A Community Health Worker is responsible for utilizing their valuable lived experiences to promote an understanding of the member’s point of view and preference. The Community Health Worker (CHW) helps address chronic conditions, preventive health care needs, and health-related social needs. A Community Health Worker will work alongside Lead Care Managers and paraprofessionals to promote client engagement, provide advocacy and case management support and triage clients to levels of care and community resources as needed.
