The Community Patient Navigator is a non-clinical, paraprofessional assistant to radiologists and clinical staff, serving as a liaison to patients and referring physician community. This role is dedicated to assuring patients receive a variety of result types as well as understand appropriate follow up measures for continuity of care. The responsibilities are wide-ranging, including communication with patient care team, identification of barriers to care, as well as detailed tracking of the patient journey.

Here is what you will need:

High School diploma, associates preferred.

Minimum of 4 years healthcare industry experience

Previous experience in high risk patient management, breast imaging or cancer care preferred

Excellent oral and written communication skills

Strong data input and quality control skills

Ability to read, write, and comprehend medical terminology

Willingness to learn specific processes and terminology

Work independently with minimal supervision

Extremely organized and detail oriented

Ability to apply common sense understanding to carry our detailed and sometimes complex written or oral instructions.

Outstanding customer service skills with both internal and external customers

Strong ability to interface with all levels of individuals

Demonstrate high level of confidentiality and professionalism

Ability to sit for long periods of time

Experience with Microsoft Office Suite applications
A Day in the Life of a Community Patient Navigator:
- Provide services and support for the following areas:
-
Accountable for communicating and documenting results to patient care team in a timely manner
-
Accountable for communicating and documenting results to patient, when that task is delegated, in a timely manner
-
Maintain a patient database to monitor results ensuring no patient experiences delays in their clinical pathway
-
Patient handoff to referring, community and joint venture partners when appropriate
-
This position may be assigned to one of the following teams: biopsy navigation or high risk navigation.

Ongoing RIS/MIS utilization

Adhere to company regulatory compliance requirements

Must be adaptable to rapid growth and amenable to team environment

Track necessary data via various verbal and written communication methods

Perform other work-related duties as assigned
