Job Summary:

White Glove is seeking a dedicated and compassionate RN Registered Nurse to join our Hospice and Palliative Care team in Manhattan. As a RN Registered Nurse, you will provide high-quality home health visiting hospice care to patients and their families, working closely with our interdisciplinary team to ensure seamless care coordination.

Responsibilities:

• Provide palliative and hospice care services to patients in their homes, focusing on symptom management, pain control, and emotional support

• Conduct initial and ongoing assessments to identify patient needs and develop individualized care plans

• Collaborate with patients, families, and interdisciplinary team members to provide comprehensive care

• Educate patients and families on disease management, treatment options, and available resources

• Maintain accurate and detailed records of patient care and progress

• Participate in team meetings and stay informed about new developments in hospice and palliative care

Requirements:

• Associate's degree in Nursing (ADN) or equivalent

• 2+ years of experience as a RN Registered Nurse, with a focus on hospice and palliative care

• Current New York State RN registration

• Active certification in Basic Life Support (BLS)
