Access Coordinator: Triage Center DMHAS- 6920

OBJECTIVE OF POSITION: To work in tandem with clinical staff, triage center, and front desk staff to assist clients in accessing care, through multiple access points.
By:
• Providing daily coordination of Open Access intakes within the CHR Service system for six clinic sites • Working with clinical staff to schedule client appointments to meet their needs. • Working to help client’s complete assessment tools needed to initial assessment • Setting up patients for appointments via telehealth sessions
