Case Manager: Hendee Road House- Per Diem (1st/2nd shift coverage)- 6544

OBJECTIVE OF POSITION: To increase the independence and self efficacy of individuals with serious and persistent mental illness through outreach support, direct assistance and training in living skills, advocacy and housing assistance. By:
• Providing individuals with serious and persistent mental illness and/or substance abuse assistance with finding appropriate and affordable housing, entitlements and other recovery resources. • Linking with appropriate treatment, rehab and recovery sources. • Providing coordination of care with collateral providers. • Assisting with, coaching and teaching skills for rehab including money management, self care, transportation, and other activities needed to live an independent and full life
