Director of Home Health (PACE Modesto)

The Director of Home Health Services oversees all of the Home Health program and associated patient care and staff providing care in this program. Responsibilities include regulatory and compliance oversight, patient and staffing assignments, coordination of care, assessment of patient needs and delivery of compliant care in participant’s home, skilled nursing facility, assisted living or group home and hospital discharge/continuity of care environments. The Director is responsible for collaborating with a variety of departments and services to insure appropriate/effective care is provided to participants with continuity of care as a primary focus.

This position will work at our Modesto PACE Center, Scheduled from 8:00am – 5:00pm.

Compensation: $155,000 – $175,000 Annual Base Salary
