Case Manager III - Heartland Bridges (evenings, full time)

The Case Manager III provides intensive case management services to an assigned caseload of Heartland Bridges clients. This role coordinates supportive services designed to meet client needs while working with them to develop a plan for wellness, recovery, and stabilization. Strong decision-making, problem-solving, and interpersonal skills are essential.

Compensation: between $19.74 and $23.54 per hour (wage is determined by total years of relevant experience) Work Hours: 40 hours per week (2:30 p.m. to 10:30 p.m.)
