MDS Coordinator PT

The MDS Coordinator is responsible for coordinating and overseeing the completion and accuracy of the Minimum Data Set (MDS) assessments in a senior living or skilled nursing community. This role ensures compliance with federal and state regulations while supporting accurate care planning, quality outcomes, and appropriate reimbursement. Working collaboratively with nursing leadership, interdisciplinary team members, physicians, and therapy providers, the MDS Coordinator manages the full MDS assessment process, including scheduling assessments, conducting chart reviews, coordinating care area assessments (CAAs), and ensuring timely submission in accordance with CMS guidelines.
