Job Description

Transitional Counselor (Part-Time)
Saturday & Sunday, 3pm–11pm (16 hours/week)

BHcare is seeking a compassionate and motivated Transitional Counselor to support residents in our Young Adult Services program. This part-time role provides direct support, skill building, and counseling to help clients build independence, stability, and a higher quality of life.

What You’ll Do:
• Assist with daily program operations and provide individualized support to residents.
• Help clients develop and implement recovery plans, reviewing progress regularly.
• Support independent living skills such as hygiene, meal planning, money management, and household responsibilities.
• Provide crisis assessment and intervention when needed, collaborating with clinical and emergency resources.
• Document services, maintain case records, and ensure accurate reporting.
• Facilitate connections to community resources, clinical services, vocational supports, and natural supports.
• Collaborate with nursing staff on medication needs.
• Ensure the safety and well-being of residents during scheduled shifts.

• Ability to support clients with co-occurring disorders using trauma-informed, person-centered approaches.
• Reliable, flexible, and committed to maintaining a safe, supportive environment.
• Must remain awake and alert at all times during scheduled shifts.
• Valid driver’s license and willingness to provide transportation when needed.

MINIMUM QUALIFICATIONS:

  • Two years of college education (human services or behavioral science preferred) or High School Diploma or Equivalent and 2 years direct care work experience in behavioral health residential care.
  • Must have and maintain a valid driver’s license. Must maintain an acceptable driving record and be insurable at a reasonable rate under the Organization’s auto insurance plan
  • Must be proficient in Microsoft 365. Must be able to navigate through electronic health records system (EHR).

 

LICENSURE /CERTIFICATIONS: 

  • Ability to obtain and maintain BLS certification
