Job Description

Summary: Advanced Practice Provider provides comprehensive medical care across urgent care, family practice, and outpatient recovery services, ensuring high-quality treatment and continuity of care for patients.

 

Urgent Care

  • Assess, diagnose, and manage acute conditions such as minor injuries, infections, sprains, and gastrointestinal complaints in a walk-in clinic environment.
  • Perform in-clinic procedures including wound care, suturing, splinting, and incision & drainage.
  • Order and interpret diagnostic imaging and lab tests; initiate timely and evidence-based treatment plans.
  • Provide same-day care with a focus on efficient patient flow, safety, and high-quality outcomes.
  • Educate patients on symptom management, follow-up care, and prevention strategies.

Family Practice

  • Deliver primary care services for patients of all ages including wellness exams, chronic disease management, immunizations, and health screenings.
  • Provide care for common chronic conditions (e.g., diabetes, hypertension, asthma), adjusting treatment based on patient response and evidence-based guidelines.
  • Promote preventive care through lifestyle counseling, risk assessment, and health education.
  • Coordinate referrals and collaborate with specialists as needed to support continuity of care.
  • Maintain up-to-date and compliant patient records within the clinic's EMR system.

General

  • Maintain accurate, timely, and confidential documentation in the clinic’s EHR system.
  • Ensure all documentation complies with organizational policies, state and federal regulations, and accreditation standards.
  • Participate in quality improvement initiatives and compliance audits.
  • Demonstrate professionalism, compassion, and respect in interactions with patients, families, and coworkers.
  • Stay current with continuing education and regulatory requirements relevant to the scope of clinic-based NP practice.
  • Uphold clinical protocols, infection control practices, and safety standards in a high-paced outpatient setting.

Qualifications:

  • Education: Family Nurse Practitioner, Family Nurse Practitioner (FNP)
  • Licenses/Certification: Full, unrestricted license in the State. ACLS, PALS. BLS
  • Experience: Minimum three years as an Advanced Practice Provider.
