St. Ann’s Community is currently looking for a dynamic care coordination manager for The Glen located on the Cherry Ridge campus in Webster. The primary purpose of your job is to oversee quality of care, case management, and overall service provided to the Residents of The Glen and Rainier Grove, to assure that the resident care meets / exceeds the requirements as specified by the NYSDOH. Creating an engaging environment with a person-centered care approach.

Job Description:
Ensures that the Memory Care SNALR community, Rainier Grove, provides residents with quality care and services, ensuring the highest level of functioning for each resident.
Dementia specific focus with collaboration in programing to meet the ever-changing needs of the residents
Ensures that the Assisted Living ALR and Enhanced Care EALR provide and environment that encourages independence and aging in place as appropriate
Maintains documentation in accordance with the Department of health Guidelines for Assisted Living.
Formal and informal evaluations.
Ongoing assessment of needs and implementation of services to meet those needs.
Focus on the total well-being of the resident which includes social, family financial, medical, mental health, key life events dietary, and spiritual.
Preadmission evaluations review, admissions and discharge in accordance with NYSDOH guidelines.
Responsible for overall development, implementation and review of Individualized Service plans of care for all Assisted Living residents.
Responsible for case management services and the coordination of medical, nursing, psychosocial and other services that may be needed and utilized by Assisted residents.
Coordinates all Agency/home care services through effective collaboration, physician referral
Responsible for ensuring that staff in Assisted Living communicate and document any change in resident’s health and/or behavior.
Attending care staff meetings and daily review of the 24-hour communication log
Part of the On-Call rotation for consultation on resident emergencies
Setting up and maintaining Resident Electronic Health Record for all residents incorporating all NYSDOH regulated documentation from admission to discharge
Audit and Quality assurance responsibilities for NYSDOH compliance as assigned by Administrator
Maintain Department of Health regulated records, census, incident report, admission discharge log Marketplace Leadership
Collaborates with Care team manager for unit based quality improvement, Helps develop and implement unit based programs to ensure quality of life and person centered care
Involved in Resident and staff satisfaction survey – goal setting for positive outcomes
Maintain community relationships – work with marketing team as directed
Maintain memberships attend ESAAL, NYSDOH and Leading age conferences as available
Candidate Requirements:
Bachelor's Degree in Social Work, Human Services or Nursing, LPN,RN, plus 3 years of related experience
Associate degree in Social Work, Human Services or Nursing, plus 5 years or related experience
Understanding of the functions responsibilities in provision of services to geriatric resident/health care and can be approved through NYSDOH
Must attend all mandatory in service training and meeting continuing education requirements
