Job Description

We are seeking a highly motivated and clinically skilled Registered Dietitian (RD) to support nutrition services within a skilled nursing facility with a short-term rehabilitation program. This role is integral to interdisciplinary care, with a strong emphasis on active participation in care plan meetings and collaboration with nursing, therapy, and medical teams.

The RD will ensure the delivery of high-quality, compliant, and resident-centered nutrition care that supports both long-term residents and short-term rehab patients.


Key Responsibilities

  • Conduct comprehensive nutrition assessments for long-term care residents and short-term rehab patients
  • Develop, implement, and monitor individualized nutrition care plans
  • Attend and actively participate in interdisciplinary care plan meetings (required)
  • Provide ongoing evaluation of residents’ nutritional status, including weight trends, intake, labs, and skin integrity
  • Collaborate with nursing, therapy, and physicians to support rehabilitation outcomes and recovery goals
  • Ensure compliance with all CMS regulations, state requirements, and facility policies
  • Provide guidance and oversight on therapeutic diets, texture modifications, and hydration programs
  • Participate in survey readiness and support during state and federal inspections
  • Educate residents, families, and staff on nutrition-related topics
  • Partner with foodservice leadership to ensure menu compliance and resident satisfaction

Qualifications

  • Registered Dietitian (RD/RDN) credentialed with the Commission on Dietetic Registration
  • Licensed Dietitian/Nutritionist in the state of [Insert State] (if applicable)
  • Experience in skilled nursing, long-term care, or rehabilitation settings preferred
  • Strong understanding of clinical nutrition, regulatory requirements, and care planning process
  • Excellent communication and interdisciplinary collaboration skills
