Job Summary

Under general supervision, adheres to the American Nurses Association (ANA) standards of professional nursing and performance, and the ANA Code of Ethics to provide care to all healthcare consumers. Applies critical thinking, evidence-based practices and competence to collaborate with the health care team to promote excellence in healthcare consumer safety and quality outcomes. Utilizes the Theory of Human Caring and Caritas in daily practice. "Jean Watson, PhD, RN, AHN-BC, FAAN, Founder / Director, Watson Caring Science Institute." Bachelor of Science in Nursing preferred. Candidates who do not hold a BSN will be considered with demonstrated experience in maternal-child health, public health, or other relevant experience. Nurse Home Visitor regularly travels to provide comprehensive community health nursing services to parents and their families eligible for program.

Essential Functions:
  • Clinical application of Nurse-Family Partnership model including but not limited to applying theories and principals of model, utilization of screening and assessment tools to identify needs of dyad to implement and evaluate interventions across 6 domains, supporting clients to establish realistic goals related to health, parenting, and life course development, connecting to community resources and referrals as appropriate. (20%)
  • Maintain a caseload of 25-30 clients having met eligibility requirements of program to improve health outcomes for eligible women and their families. (15%)
  • Home visits performed in accordance with Nurse-Family Partnership program model. Occasionally, visits conducted via telehealth. (30%)
  • Documentation of clinical notes and data entry in a timely manner. (20%)
  • Ensures coordination with team through reflective supervision and case conferencing. (10%)
  • Participates in community outreach, coalitions, and other activities representing the NFP department. (5%)

Education:
  • Associate's Degree Completion of an accredited nursing education program that satisfies the licensing requirements of the Colorado State Board of Nursing for Registered Nurses. Required or
  • Bachelor's Degree Completion of an accredited nursing education program that satisfies the licesning requirements of the Colorado State Board of Nursing for Registered Nurses.

Work Experience:
  • 1-3 years Clinical experience with diverse populations in a health care setting including hospital, clinic or home health providing patient education, case management and/or care coordination. Required

Licenses:
  • BLS-Basic Life Support (BLS/CPR) - AHA - American Heart Association or American Red Cross Required and
  • RN-Registered Nurse - DORA - Department of Regulatory Agencies Required

Knowledge, Skills and Abilities:
  • Adherence to Standards of Professional Nursing Practice/ Process / Performance [ANA Standards addendum- signed] and accrediting body standards.
  • Ensures all duties, responsibilities and competencies are conducted in a manner that is effective and appropriate to patients/clients to who care/service is being provided.
  • Demonstrates knowledge and applicability of the principles of growth and development over the life span, as well as demonstrating the ability to assess data reflecting the patient’s status and interpreting appropriate cultural information of the patient(s) to whom care/ services is being delivered/provided.
  • Employee has completed and met their clinical competency standards.
  • Ability to use medical equipment and instruments to administer patient care.
  • Skilled in maintaining effective working relationship with patients, employees, and the public.
  • Ability to react calmly and effectively in emergency situations.
  • Ability to interpret adapt and apply guidelines and procedures.
  • Ability to communicate clearly.
  • Microsoft Office Suite required.
  • Experience with EPIC documentation preferred.
