Description: The Acclaim Physician Group is currently seeking a full time (Night) Acute Care Provider to join our group and care for patients in our 32 bed Clinical Decision Unit. We are looking for a provider who is not afraid of a challenge and enjoys working in a fast-paced setting. The role would entail patient care responsibilities in the Clinical Decision Unit (CDU) located at the main JPS Health Network campus just south of downtown Fort Worth. Join our collegial and team focused group of physicians and APP’s in our 32 bed CDU. Specific Details: CDU: · 12-hour shifts · Average of 20 CDU admissions per day · 1 Advanced Practice Provider and 1 Physician per shift · Working location for this position is at the main JPS Health Network campus only · JPS is a 582-bed community hospital consisting of a Level 1 Trauma Center and a 36 bed Closed ICU Duties and Essential Job Functions: 1. Collaborates with the physician in overseeing and coordinating the multidisciplinary care delivered to all patients, to optimize quality patient care. 2. Reviews patient medical charts to assist physicians with diagnosis, treatment and care management. 3. Monitors guidelines for the management of care and for ensuring that the services are delivered in compliance with the guidelines established by JPS Health Network and Acclaim Physician Group. 4. Provides care education to patients and families. 5. Provides evidence-based and compassionate care to all patients assigned to improve patient outcomes and increase patient satisfaction scores. 6. Maintains a collaborative environment with providers, clinical staff, and support staff. 7. Supports academic, educational and research endeavors. 8. Attends Performance Improvement Programs and Peer Review committee meetings; and is an active participant in the process. 9. Performs other job-related duties as assigned.
