APP - Family Medicine Float Pool

Job Summary: Acclaim Multi-Specialty Group has an exciting opportunity for experienced Advanced Practice Providers to join the Community Health/Primary Care Float Team. The Float Team supports 11 JPS Patient-Centered Medical Homes throughout Tarrant County, collaborating with Family Medicine, Internal Medicine, Pediatrics, and Advanced Practice Professionals. We are looking for providers who thrive in dynamic environments, enjoy a challenge, and can seamlessly transition between different clinic settings. If you love working in a fast-paced outpatient setting and are enthusiastic about serving the residents of Tarrant County, this position is for you! --- Essential Duties and Responsibilities: · Work under physician supervision to provide comprehensive outpatient care. · Perform physical exams, diagnose illnesses, order and interpret tests, and develop treatment plans. · Educate patients on preventative care and prescribed treatments. · Monitor care guidelines to ensure compliance with JPS Health Network and Acclaim Physician Group standards. · Make appropriate patient referrals as necessary. · Maintain a collaborative environment with physicians, clinical staff, and support teams. · Support academic, educational, and research initiatives. · Participate in Performance Improvement Programs and Peer Review meetings. · Other duties as assigned.
