Job Description

This practitioner is expected to collaborate with physicians, nurses and other health professionals to design, implement, monitor and evaluate pharmacotherapy to ensure, safe, and economical patient care; may also be responsible for facilitation of patient transition from hospitalization to post discharge disposition. Other duties include: monitoring drug therapy, distribution (preparation, compounding, manufacturing and dispensing of drugs/medicines on the basis of prescriptions or orders), and education of pharmacy students and other health professionals.

Education And Credentials

BS - Bachelor of Science Pharmacy required. PHARMD -Doctor of Pharmacy preferred. Required if applying without the preferred post-graduate residency training as defined in the job posting, then must have minimum post-graduate residency training and additional practice experience (in area of intended practice and as defined in the job posting) providing direct patient care clinical services. Additionally, must have board certification (s) (in intended area of intended practice and as defined in the job posting) upon hire. Previous Health systems clinical pharmacy practice experience preferred. If preferred post-graduate residency training and clinical experience (in area of intended practice and as defined in the job posting) has been completed, then certification(s) (as defined in the job posting) is (are) required within 12 months of hire or upon earliest eligibility and allowance to sit for respective exam(s). Pharmacist upon hire required.
