Part-Time Licensed Practical Nurse (LPN)

Role will work as a member of a multi-disciplinary team to deliver comprehensive, person-centered, integrated services to the community with a focus on medication tracking, ordering, handling and auditing. The LPN plays a critical role in medication management and patient care by supporting providers and clients and are also integral members of the treatment team. LPNs work with client access specialists, and non-clinical staff to ensure the workflows are efficient, the client experience is positive and medication safety and accuracy is prioritized. Additionally, this role includes interaction with clients during medication pass times, offers general assistance with medication self-administration, supports medical issues or concerns as a key member of the integrated treatment team.
