Position: Physical Therapist Case Manager (PT CM) for Home Health Salary Range: $91,000 - $110,000 annually, depending on experience Service Area: North Central. Plus mileage reimbursement of 72.5 cents per mile. Schedule: Monday – Friday 8:00am-5:00pm (weekend rotation: every 10th weekend / holiday rotation: 2 per year / no on-call rotation)
