Job Summary
The Referral Outreach Coordinator increases organization visibility, builds strategic partnerships with community resources, and manages the end-to-end referral process. This role combines business development, marketing, and administrative duties to meet monthly or quarterly referral targets.
Initially this position will be part-time, in-person in Crown Heights, Brooklyn. We hope to grow it to a full time, hybrid position.
Key Responsibilities
- Referral Generation & Outreach (50-60%): Identify, cultivate, and maintain relationships with referral sources (e.g., physicians, schools, non-profits, community agencies).
- Relationship Management: Act as the primary liaison, providing education on services, presenting at community meetings, and strengthening partnerships.
- Documentation & Tracking: Maintain a CRM (e.g., Monday) to track referral metrics, outreach activities, and follow-ups to measure performance against goals.
- Patient/Client Engagement: Contact prospective clients to inform them of services, conduct intakes, and facilitate the enrollment process. (A lot of phone calls, emails and online meetings!)
Qualifications & Skills
- Experience: Typically 2+ years in referral coordination, intake, community outreach, sales, or healthcare administration.
- Communication: Exceptional verbal, written, and public speaking skills for presentations and partnership meetings.
- Software Proficiency: Must learn Instantly, Monday and other platforms.
- Organization: Strong attention to detail with the ability to manage high-volume, time-sensitive tasks.
