Reentry Health Home Plus Care Manager

Health Home Plus (HH+) is a model within the Housing Works Health Home Care Coordination program that provides a higher level of care, for specialized populations (SMI and HIV+) of Health Home members. The higher level of care is supported by specific caseload ratios, level of service provision, staff qualifications, and staff training. The CM works in facility that provides time limited transitional housing and support services to individuals reentering the community following periods of incarceration or other justice system involvement and will participate in the client’s care as part of the integrated care team at the Fresh Meadows Hotel site.
