$101,254.00 - $131,685.00The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
Responsibilities:
GENERAL PURPOSE:Maintains 24 hour-a-day accountability for all patient care and nursing activities on assigned outpatient unit. Oversees and directs unit activities including standards development, human resource management, assessment of resource needs and utilization, staff development and communication within nursing practice as well as among other disciplines and departments. Oversees all performance improvement activities at the unit level and participates in committees at all levels within the institution.

Managing a recognized Patient-Centered Medical Home (PCMH), will lead a dedicated clinical team of RNs, LPN, Nurse Navigators and ambulatory technicians, while driving the care coordination, quality improvement, and population health initiatives that define our model of care. Will partner closely with providers to manage chronic disease workflows, close care gaps, and track key quality metrics, all while overseeing staff hiring, onboarding, and performance. Candidates should hold an active NYS RN license (BSN required; MSN required within 5 years) with at least three years of clinical experience, including some supervisory background, and a solid understanding of PCMH principles and value-based care.
