Description
Responsible for the coordination of patient shifts and visits, maintenance and upkeep of scheduling records and logbooks and patients or family members. This includes accurate and timely communication of scheduling changes between office and field staff and aide roster.
Essential Functions
- Promote /exemplify the Company's mission, vision, and values always.
- Ensure timely staffing and scheduling visits for field staff in their appropriate areas per physician orders, insurance authorization, and/or patient/client requests.
- Ensure timely follow-through with staff, patients/families, and physicians, which may include but not be limited to the Start of Care (SOC) report, Schedule Calendar Report, emails, faxes, labs, etc.
- Maintain a current client roster with necessary information.
- Ensure caregivers have met all HR requirements and possess the necessary skills for their assigned patients.
- Communicate effectively with clients, their families, team members, and other healthcare professionals.
- Promote the Company's financial success by maintaining proper caregiver pay rates and limiting overtime hours.
- Demonstrate commitment and professional growth by participating in in-service programs and maintaining/improving competency.

- Create, maintain, and adjust daily and weekly schedules for hospice clinicians and support staff
- Coordinate visit coverage across disciplines to support timely and compliant patient care
- Respond to scheduling changes, staff callouts, and urgent coverage needs
- Support compliance with visit frequency, timeliness, and Plan of Care requirements
- Communicate schedule updates, changes, and expectations to clinical staff and leadership
- Monitor scheduled versus completed visits and escalated coverage gaps or risks to the ADPCS
- Support interdisciplinary workflow coordination in collaboration with branch leadership
- Assist with documentation tracking and scheduling-related reporting as directed including HOPE visit requirements
Experience
- At least one (1) year experience in a general office environment.
- Preferred, Health care experience.
- CNA, preferred
Skills
- Ability to communicate verbally and in writing effectively.
- Computer skills.
- Must read, write, and comprehend English.
Education
- High school diploma or equivalent.
Licensure/Certification
- Current driver’s license in good standing. The employee is responsible for renewing their driver’s license before it expires to continue employment.
Physical Requirements
- Prolonged sitting, standing, and walking are required.
- Ability to always handle stressful situations in a calm and courteous manner.
- Requires working under some stressful conditions to meet deadlines and Company needs.
Environmental/Working Conditions
- Works primarily in an office environment.
- Some exposure to unpleasant weather.
Machinery/Tools/Equipment Requirements
- Reliable transportation and auto liability insurance.
- Computer and basic office equipment.
- Will be in an on-call rotation.
