North Suffolk is looking for LPNs to join our team! As an LPN for North Suffolk Community Services you will provide support, medical & regulatory oversight (DDS regulations and MAP policy) to residential programs. The LPN is responsible for overseeing health care services provided to individuals. Must comply with applicable regulatory requirements and standards in the performance of nursing duties. Responsible for all medical follow up and supervision of CNA staff. 

Full and Part Time (24 hours) position available.

This position may be assigned to one specific program or visit multiple programs, depending on programmatic and client needs.

What you'll do:


  • Assist with health care services provided to individuals supported. Participate in medical discharge meetings at hospitals.
  • Obtain reports and approval of treatment plans from PCP/Specialists.
  • Review seizure and medical incident reports and make recommendations to program staff and assist with any follow up as necessary.
  • Participate in managers’, clinical and staff meetings to review health care needs and global health care planning with all staff.
  • Provide medical training in, but not limited to, Diabetes, Nutrition, Seizures, Oxygen and Vital Sign training to all staff.
  • Conduct MAP audits at site yearly and follow all Map policies and procedures.
  • Perform direct support related tasks in emergencies or as requested by supervisor.
  • All other duties assigned by supervisor.


What you'll need:


  • Education/Licensure: 
    • Licensed Practical Nurse, registered with the MA Board of Registration in Nursing.
  • Experience: 
    • Minimum of one year of experience in a community health setting. Prior experience working with developmentally disabled adults and/ or mental health.
  • Skills: 
    • Must be, or become, MAP certified within five months from date of hire (training provided)
    • Must be, or become, First Aid/CPR certified within 5 months of hire
