Job Description
Job Summary
Provides and coordinates patient care across all phases of the transplant continuum. Serves as a fully effective member of the multidisciplinary transplant team. Provides the full range of professional practice in the delivery of patient care and in participation of the development of strategies to improve patient outcomes, satisfaction with service, and cost effectiveness. Accountable to patients and families for the processes and outcomes of care during the pre- and post-transplant phase of care.
Primary Responsibilities
· Coordinates care delivery of transplant patients. Designs pre-transplant medical procedures and diagnostic testing for transplant evaluation. Coordinates scheduling of all post-transplant medical procedures, diagnostic testing and clinic visits deemed necessary by the provider.
· Collects and reports on comprehensive data pertinent to the patient’s health status and situation including test results. Recognizes abnormal findings and initiates referrals accordingly for pre- and post-transplant patients. Provides medical and psychosocial information to the Organ Procurement Organization and transplant surgeon for matching and allocating organs.
· Assesses and addresses patient safety concerns.
· Identifies risks in processes, procedures, devices and communicates accordingly.
· Addresses the availability and accessibility of specialized care required for transplant patients and families.
· Considers factors related to safety, efficacy, cost, and impact in the planning and delivery of care. Actively participates in multidisciplinary discussions regarding patients plan of care.
· Provides continuing transplant patient education appropriate to the patient’s developmental level, learning needs and cultural values and beliefs.
· Assists in the monitoring, documentation and evaluation of data-driven quality assurance and performance improvement programs. Supports the evaluation of and changes to improve outcomes such as readmissions, reduction in ED visits, patient and graft survival, patient satisfaction, patient education, and patient progress through phases of care.
· Participates in performance improvements activities; quality improvement and patient safety activities; assists in maintaining compliance with The Joint Commission, CMS, UNOS regulations (or Foundation for the Accreditation of Cellular Therapy for BMT) and other internal and external regulatory standards.
· Completes all required forms for donors and pre-and post-transplant patients as applicable.
· Participates in and supports regulatory agency audits.
· Adheres to clinical documentation guidelines, comprehension of outcomes data, engagement in performance improvement activities and commitment to standard work.
· Perform all other duties as assigned.
Work Experience
Education & Experience - Required
· Bachelor’s degree
· 3 years RN experience required with 1-year previous experience in critical care or 2 years adult applicable bedside nursing.
· RN licensure in the state of Maryland,
· Certified Clinical Transplant Coordinator (CCTC) or Blood & Marrow Transplant Certified Nurse (BMTCN) required within 2 years of hire date.
· Active BLS certification
Education & Experience - Preferred

Knowledge, Skills, & Abilities
· Demonstrates awareness of performance standards required by regulatory bodies.
· Maintains up-to-date knowledge of transplant regulatory policies and procedures.
· Maintains knowledge of the principles of ethical practice and have resources available to evaluate the merits, risks, and social concerns of transplantation.
Work Environment & Physical Demands
Work Environment: Medical Related Facility - climate and lighting controlled, clean, but with biohazards and medical equipment hazards.
Physical Requirements: Strenuous-regular and considerable walking, climbing or standing for extended periods, regular lifting, pushing or pulling of objects, materials, tools, equipment, and/or patients weighing up to 50 pounds.
Fine Manipulation Skills Requirements: Extensive-continuous and intense use of fine manipulation skills with both hands in performing measuring, testing, technical procedures, crafts activities, et.al.
Corrected Vision Requirements: Normal Range Near, Far and Peripheral Vision.
Speaking Attribute Requirement: Communicates effectively in person and by remote device.
Hearing Attribute Requirement: Hear and decipher person speech in person.
