Job Description
Key Responsibilities
-Perform comprehensive patient evaluations and develop individualized treatment plans -Provide ongoing therapy services to support patient recovery and mobility -Collaborate closely with the hospitalist team, nursing staff, and case management to determine appropriate discharge recommendations -Deliver care across multiple settings, including inpatient units and the Emergency Department
