Job Summary
The Case Manager is responsible for coordinating the delivery of quality resident care in our Memory Care neighborhood. This role requires strong planning and organizational skills and involves applying advanced nursing clinical skills in maintaining records, preparing reports, teaching nursing-related skills, and fostering effective working relationships with others. The Case Manager will be actively involved in both direct and indirect care for our residents. 
 
The Responsibilities
  • Provide comprehensive resident care coordination
  • Oversee the medication management program, including delegation of medication setups to properly trained and certified team members
  • Assist in training team members on resident care procedures and protocols
  • Oversee resident wound care
  • Act as a primary resource for staff questions related to resident care
  • Communicate with residents and family members regarding resident progress, care plans, and health updates
  • Record and monitor all progress of residents, ensuring timely communication with physicians for medical orders and treatment
  • Provide treatments and document physician orders appropriately
  • Stock and prepare medication reminder carts, ensuring safe and secure medication distribution
  • Order medications for residents in accordance with current policies and procedures.
What You'll Bring
  • Required: Current RN or LPN license in good standing from the state of South Dakota.
  • Experience in assisted living or long-term care preferred.
  • Strong leadership, communication, and interpersonal skills.
  • Effective decision-making and problem-solving abilities.
  • High level of initiative, time management, and ability to multitask.
  • Competency in working with electronic medical records and online nursing assessments.
  • Ability to lift 50 pounds, push/pull 60 pounds, and carry 15 pounds with two hands.
