What You'll Do
As a Transitions Hospice Night Nurse, you'll serve as a critical support member of the interdisciplinary care team during overnight hours — assessing, treating, and communicating patient needs with clinical confidence and genuine compassion. Your responsibilities include:
- Delivering professional nursing care across the full nursing process within your scope of practice
- Conducting thorough patient and family assessments and contributing to individualized care plans
- Completing all visit types as needed to meet overnight patient and family needs
- Administering medications and treatments per physician orders
- Identifying nursing diagnoses from health assessment data and responding with sound clinical judgment
- Recording accurate, timely clinical notes including pain and symptom management outcomes
- Communicating changes in patient condition to physicians and obtaining orders as required
- Coordinating with community health partners to support continuity of care
- Ensuring equipment and necessary services are in place for patients
- Maintaining and growing your clinical skills through continuing education and in-service participation

What You Bring
- Active RN or LPN license in the state of employment
- Graduate of an accredited school of Nursing.
- Clinical experience in hospice, home health, or a related setting preferred
- Comfort and composure working independently during overnight hours
- Strong assessment skills and sound clinical judgment in high-pressure moments
- A calm, reassuring presence for patients and families during nighttime distress
- Excellent documentation habits and communication skills
- Valid driver's license and willingness to travel locally
