Long Term Care Functional Screener

Lutheran Social Services of WI & Upper MI is currently seeking a Long-term Care Functional Screener to join our IRIS Consulting team in Milwaukee, WI. Hours for the position are primarily Monday through Friday first shift, but require some flexibility to support the needs of the individuals supported with a strong commitment to enabling people to be in control of their lives.

Functional Screeners performs professional work involving the administration of Long Term Care Functional Screens (LTC FS) to participants enrolled in the IRIS (Include, Respect, I- Self Direct) program, in order to establish a level of care and functional eligibility for all publicly funded long-term care programs serving adults in Wisconsin.
