Second Chances Comprehensive Services is a community-based behavioral health organization founded in 2008, dedicated to supporting at-risk youth, adults, and families through comprehensive mental health services. Quality Assurance & Clinical Case Manager (Hybrid)

Second Chances Comprehensive Services is seeking a licensed or provisionally licensed LPC or LCSW for a unique hybrid role combining direct clinical care with leadership in quality and compliance. This position is ideal for a clinician who enjoys working with clients while also improving systems, guiding staff, and strengthening service delivery across programs.

You’ll play a key role in both client care and organizational impact—supporting individuals and families while helping ensure services meet the highest clinical and regulatory standards.


What You’ll Do

Clinical & Case Management

  • Provide counseling and case management services to adults, children, and families in community-based programs
  • Conduct biopsychosocial assessments, diagnostic impressions, and ongoing clinical evaluations
  • Develop and maintain Individualized Service Plans (ISPs) in collaboration with clients and care teams
  • Deliver trauma-informed, person-centered care aligned with best practices
  • Ensure compliance with HIPAA, ethical standards, and licensure requirements

Quality Assurance & Compliance

  • Conduct clinical record reviews and documentation audits to ensure accuracy and compliance
  • Review and approve assessments, ISPs, progress notes, and supervision documentation
  • Ensure alignment with VDBHDS, DMAS, and applicable state/federal regulations
  • Identify trends, gaps, or risk areas and present findings to leadership
  • Recommend and support corrective actions and quality improvement initiatives

Supervision & Leadership

  • Provide clinical supervision to assigned QMHP staff
  • Lead individual and group supervision sessions, offering guidance and professional development
  • Review documentation and provide feedback to maintain high clinical standards
  • Support staff in adhering to agency policies, procedures, and ethical practices

Process Improvement & Risk Management

  • Help strengthen workflows, systems, and internal controls
  • Partner with leadership to improve service quality, efficiency, and compliance
  • Contribute to a culture of accountability, continuous improvement, and ethical care delivery

What We’re Looking For

  • Active LPC or LCSW license or provisional license in the Commonwealth of Virginia
  • Experience in community-based mental health services
  • Strong knowledge of clinical documentation, service planning, and compliance standards
  • Experience with (or interest in) supervision and quality assurance
  • Ability to balance direct care with administrative and leadership responsibilities

Qualifications

Required

  • Active and unrestricted LPC or LCSW license or provisional license in the Commonwealth of Virginia.
  • Master’s degree in Counseling, Social Work, or a closely related mental health field.
  • Minimum of 2–3 years of clinical experience in community-based mental health services.
  • Demonstrated knowledge of VDBHDS/DMAS regulations, Medicaid documentation standards, and clinical compliance requirements.
  • Prior experience providing clinical supervision to QMHPs or other behavioral health staff.
  • Strong auditing, analytical, and documentation review skills.
  • Excellent written and verbal communication skills.

Preferred

  • Experience in quality assurance, utilization review, or compliance monitoring.
  • Familiarity with electronic health record (EHR) systems.
  • Experience developing quality improvement plans and performance metrics.
  • Strong organizational skills with the ability to manage multiple priorities independently
