Job Description
The Housing Case Manager supports the Day 1 Family Fund’s mission to stabilize families experiencing homelessness or housing insecurity by providing direct intensive case management, housing navigation, and coordinated service support. This role focuses on assessing family needs, developing individualized housing plans, and guiding households through the process of securing safe, stable, and sustainable housing. The Housing Case Manager works closely with families to build trust, identify barriers, and connect them to emergency assistance, transitional housing, and long term housing solutions. Through consistent case management, advocacy, and trauma informed support, the Housing Case Manager plays a critical role in reducing obstacles to housing and coordinating essential services.
