Job Description
Assesses, plans, develops, coordinates, and directly provides flexible treatment, rehabilitation and support services to individuals who have the most serious symptoms of mental illness and who consequently have the greatest difficulty with basic daily activities such as keeping themselves safe, caring for their basic physical needs, maintaining a safe and affordable place to live, maintaining sobriety, and other activities of daily living. Rather than brokering services, the team itself is the service delivery vehicle, and assumes direct responsibility for providing the specific mix of medically necessary services required by the persons-served, if there is a need. This position promotes recovery-based, person-centered, and culturally competent practices.
