As a Home Health Occupational Therapist, you’ll work one-on-one with patients in the comfort of their homes to restore function, promote safety, and help individuals achieve greater independence with daily living activities. This role offers autonomy, flexibility, and the opportunity to build strong patient relationships while collaborating with an interdisciplinary care team.
Responsibilities
As an Occupational Therapist, you will:
- Perform comprehensive clinical assessments to identify patient needs and determine appropriateness for occupational therapy services
- Develop, implement, revise, and complete individualized Plans of Care in a timely manner
- Document evaluations, progress notes, physician orders, and all clinical records accurately and efficiently
- Provide therapeutic interventions to improve strength, mobility, coordination, functional independence, and safety
- Assess and address activities of daily living (ADLs), self-care, work, leisure, and cognitive-perceptual functioning
- Recommend and educate patients on adaptive equipment, environmental modifications, and home safety strategies
- Fabricate and maintain adaptive equipment and hand splints as appropriate
- Collaborate with the interdisciplinary group (IDG) to coordinate patient care and achieve optimal outcomes
- Apply infection control practices and standard precautions during patient care activities
- Maintain effective communication with patients, families, physicians, and care team members
- Participate in continuing education and maintain compliance with agency policies and licensing requirements
Qualifications
- Current and valid Occupational Therapy license
- Minimum of 1 year of occupational therapy experience preferred
- Home health experience preferred
- Current CPR certification
- Valid driver’s license, reliable transportation, and current auto insurance
- HomeCare HomeBase experience preferred
Physical Requirements
- Ability to stand, walk, sit, climb, crawl, squat, and kneel for extended periods
- Ability to lift, carry, push, or pull up to 100 lbs.
