How you’ll make an impact in this role
- Provide direct patient care, including triage, blood draws, wound care, and execution of physician orders while monitoring and documenting patient conditions.
- Develop, implement, and evaluate individualized care plans in collaboration with the interdisciplinary team (IDT).
- Coordinate care across disciplines and support discharge planning to ensure continuity of care.
- Educate patients, caregivers, and staff while participating in team and care plan meetings.
- Participate in on-call rotation and travel to patient residences or care facilities; part-time (20 hrs/week) with flexible schedule and documentation options.
What minimum requirements you’ll need
Licensure / Certification / Registration:
- BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
- Drivers License credentialed from the Illinois Department of Motor Vehicles (IL-DMV) obtained prior to hire date or job transfer date required.
- Registered Nurse credentialed from the Illinois Board of Nursing obtained prior to hire date or job transfer date required.
Education:
- Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
What additional preferences we're seeking
- 2 years of hospital or home health experience strongly preferred.
