Principal Objective

Graham Windham's Care Coordination program seeks a Full Time Data & Operations Associate to primarily support the program’s administrative requirements. This role is pivotal to the program; this staff member will efficiently attend to a range of responsibilities, will look for and implement efficiencies, and will initiate new tasks and functions as needed. 

Primarily responsible for managing Graham Article 29i’s quality control measures specifically in the areas of data base management and operational functions. This position requires the ability to collect and review data, generate reports, create and reinforce fiscally responsible practices and communicate effectively with Article 29i Care Coordination program and other Graham departments that are invested in the operational function of the program. This individual must possess strong organizational, analytical, communication and problem-solving skills.

 

Duties & Responsibilities (but not limited to):

  • Coordinate administrative clerical and inter-departmental communications; Provide administrative support to the Article 29i team. Key areas include managing all data entry requests, etc
  • Work closely with Director of Care Coordination to maintain all programs Electronic Health records
  • Develop proficiency and become the subject matter expert for the departments electronic health records for the Care coordination program
  • Sustain close communication with Care Coordination Program Assistant and other Care Coordination staff to maintain the fidelity and accuracy of information in the databases. This includes inputting any necessary vital data. 
  • Complete necessary documentation and data collection in a timely manner.
  • Prepare and read materials for the Department. This includes, but is not limited to, the typing of administrative correspondence, management reports, and data summaries.
  • Process mail for the department, handling correspondence
  • Manage program’s office calendar and track staff schedules
  • Attend database meetings scheduled by Graham’s Program, Performance and Evaluation department and other meetings as needed
  • Ensure confidentiality of data collected and stored.
  • Complete other tasks as assigned by the Program Director, and facilitates; other projects as assigned.


 Minimum Requirements

  • Associates Degree
  • Familiarity with use of computers and software systems including spreadsheets, database and word processing
  • Good Interpersonal skills and ability to work with people from various demographics and disciplines
  • Strong typing and data entry skills
  • Advanced computer skills
  • Ability to multi-task
  • Strong organizational skills
  • Excellent proofreading and editing skills
  • Ability to work independently as well as part of a team
  • Ability to meet deadlines and demonstrated full attention to detail
